After Skull raped and killed two of the four female police officers he kidnapped, the death toll in Manila increased, prompting the police general to send Task Force Agila on a new mission to find the serial killer behind the excessive killings.
Many are those in the Special Anticrime Task Force who think Skull is behind the evil act. Meanwhile, a new face is introduced when Cardo escorts Major Romero to her house.
On his way, Cardo witnesses a major crime in the middle of the streets, and he shoots to kill a man, believing he is the serial killer his team has been searching for. However, upon further investigation, Robert Smith turns out to be innocent, though he has also been killing the sluts he sleeps with.
The serial killer victims are rather males and not women. Jacob, whose brother Ronald passed away in the hands of a woman named Maureen, also receives a hint that his brother’s killer is roaming at a public market, and he makes a fast move to kill her.
Fortunately, Jane escapes and seeks help from Cardo at Camp Crame. Cardo then makes a tough decision to secure her from her pursuers. Unknown to Cardo, Jane is suffering from dissociative identity disorder (DID), and her other persona, Maureen, is plotting to entrap him, while the other persona, who is believed to be her protector against men, is also ready to kill Cardo.
Living in Cardo’s neighborhood, Jane secretly sneaks out to stab and kill men in the middle of the night. Getting a new lead that the serial killer might be the one who killed Ronald, a man who died mysteriously, Cardo and his team conduct new research, which leads them to Bulacan, where they learn about a lady named Maureen, who is the culprit behind Ronald’s death.
However, the revelation leads to flashback scenes of how Jane, through a traumatic experience in the past, developed four distinctive personalities. Those personalities helped her to deal with the pains associated with the abuse she suffered from her mother, father, and rapists when she worked in a bar with her mother, who is mostly a hook-up woman.
Cardo requests a sketch of the woman who is believed to have killed Ronald, and surprisingly, he witnesses a familiar face as the real serial killer. How will the scene unfold in Brothers concerning a serial killer who is suffering from a mental illness?
