The first half ended in a tense goalless stalemate, but it was Berekum Chelsea who looked intent after the break, scoring the game’s first goal through Kelvin Obeng in the 80th minute.
Despite being reduced to ten men in the 67th minute, the Bibires never gave up, picking up maximum points to improve their prospects of remaining in the league.
Berekum Chelsea won their third game in a row after going winless in their previous three Premier League matches.
Following the loss, Kotoko will face increased pressure in the race for the Ghana Premier League crown.
Medeama, who upset Great Olympics last Saturday to reduce the gap, is trailing the Porcupine Warriors.
With four games remaining, Kotoko is six points clear of the Yellow and Mauves.
Chelsea now sits in 9th place with 43 points, while Kotoko remains at the top of the table with 56 points.
